Disqualification Form must be done online only:
This report must be filed with KSHAA and Tri-State Officials Association assigning
secretary. Disqualification Forms are for the exclusive use of the Kentucky
High School Athletic Association officials. This link will take you
directly to the KHSAA site. It is the responsibility of the umpire to report
immediately (within 24 hours) in writing any unsportsmanlike conduct or
action on the part of coaches, players, school administrators and or
spectators that results in the ejection of a coach, player, administrator,
or spectator from a contest. The timely submission of these reports,
in writing, is required and officials must use the online form.
Submission in this manner will automatically forward a report to both
schools and to KHSSA. Umpires must include
the first and last name of the offender! If you do not have
access to the internet, someone in the association does and get the
disqualification reported.
To download any of the files listed below, right
click the web link (i.e. School Notification Letter) or file and select 'Save target as'.
You will then asked for a location to save this document. It will
automatically save to disk. Once this is complete you can open in your
word processing software. Otherwise, the file will open up in
a browser window.
